Consent for Treatment & Accuracy of Electronically Submitted Application forms

Medical Webform

The medical form is to ensure that we are aware of any potential medical problems that may arise during the tour. In some special cases (and for all fulltime applicants) we may ask that a doctor evaluate your medical condition before participation in a CTI tour. We reserve the right to not place an individual on a team if a medical condition could compromise the safety of the individual while overseas.

Physician's Evaluation (Fulltime only)

This is an attachment we sent with the "CTI Guide and MPH Forms" email.  This portion of the attached PDF needs to be physically filled out and signed by a licensed physician before being returned to us.

The medical section of Fulltime MPH forms is to ensure that we are aware of any potential medical problems that may arise during the tour. We require all Fulltime applicants to have a doctor's evaluation of their medical condition as part of the application process.



$50 deposit

For summer team members, a $50 deposit is due with the MPH forms to secure your position on a team and start your fundraising.

For fulltime team members, the $50 secures your position.

The deposit can either be mailed to the CTI Office (made out to CTI Music Ministries with your name on the memo line) or it can be donated online at ctimusic.org/donate and designated in support of yourself.

Passport

If you already have a passport:
  1. Ensure that the expiration date is more than six months past the date of your return from CTI - many countries will not allow you to enter if your passport expires within 6 months of your return flight.
  2. Ensure that you have signed your passport where indicated.
  3. Scan and email the identification page to us (the one with your photo and information on it.)  Make sure all written information (your legal name, expiration date, passport number, etc.) is clearly visible in the scan.  We will need this information prior to your arrival in order to purchase your ticket for international travel.
  4. Unless we specifically tell you otherwise, you can hang on to the actual passport until you arrive for training.  Just make sure you don't forget it!

If your passport will expire within 6 months of your return, you will need to renew it right away:
  1. Scan and email the identification page of your expiring passport to us before you send it in to be renewed.
  2. Let us know you have applied for a renewal.
  3. If you are renewing within 8 weeks of your arrival to CTI, you will need to apply for expedited processing in order to ensure you receive your new passport in time. 
  4. As soon as your new passport arrives, sign it, and then scan and e-mail the identification page to us.
  5. Unless we specifically tell you otherwise, you can hang on to the actual passport until you arrive for training.  Just make sure you don't forget it!

If you do not have a passport yet, you will need to apply for one right away:
  1. Scan and email the application page to us after you fill it out, but before you submit it.  Make sure all written information (your legal name, etc.) is clearly visible in the scan.  We will need this information prior to your arrival to purchase your ticket for international travel, and we must list your name exactly as it will appear on your passport.  Accuracy is critical: if your ticket does not match your passport exactly, you will not be allowed to travel, and correcting a misspelled, incomplete or erroneous name on a ticket is costly.
  2. If you are applying for a passport within 8 weeks of your arrival to CTI, you will need to apply for expedited processing in order to ensure you receive your new passport in time. 
  3. As soon as your passport arrives, sign it, and then scan and e-mail the identification page to us.
  4. Unless we specifically tell you otherwise, you can hang on to the actual passport until you arrive for training.  Just make sure you don't forget it!

As stated in the Guide to the CTI Commitment, fees for passport application, renewal and any expedited processing are your responsibility.  However, these expenses can be reimbursed if you raise additional funds.  Save your receipts and consult the Guide we sent you for further details.  As always, if you have any questions, give us a call!



Fundraising Letter

We attached a sample fundraising letter to the email "CTI Fundraising Packet".  You can edit this letter, write your own letter, or just insert your own photo and personal information in the sample.  We must approve your final letter before you send it to your supporters to ensure it accurately reflects the ministry of CTI.



Certificate of Immunization

This is an attachment we sent with the "CTI Training Preparation" email. You need to sign and return this form indicating that you have completed this requirement.  It is not necessary to send any doctor’s certifications.

As stated in the Guide to the CTI Commitment,  you are required to obtain our standard non-travel-specific immunizations prior to arriving for training. Once we finalize your team placement, we'll send you recommendations for travel-specific immunizations which you may consider and pursue at your own discretion.



Host Home Letter

This item is further explained in an attachment to the email "CTI Training Preparation"

During training you will be staying with an area host family. We'd like you to write a letter introducing yourself to them and include a photo so that they can be praying for you.  Further instructions and a sample letter are included in the attachment to this email.  You can email this to us, or write one by hand and send it to us in the mail.

Travel Plans Webform

You were sent a personalized link to this webform in the email "CTI Training Preparation".  The attachment to that email contained a sheet with specific instructions and dates pertaining to your domestic travel to and from Willmar.  A generic version of that form (without specific dates) can be found here.



Allergies and Dietary Preferences Webform

You were sent a personalized link to this webform via the email "CTI Training Preparation".  As we prepare for your arrival, we want to do our best to serve your needs through mealtimes and your housing arrangement during training.

While we will make every effort to accommodate your preferences here in Willmar, you should be prepared for the fact that you may have limited control over your environment overseas. We'll inform your international partners of any life-threatening or severe allergies, but you are encouraged to be flexible regarding less severe areas of preferences or mild discomfort.



Motor Vehicle Record (select Fulltime team members only)

If you are 23 or older, you may be asked to be one of the drivers for your fulltime team.  Our insurance company requires us to provide them with a copy of your motor vehicle report. You can get this report wherever you would apply for or renew your driver’s license.

Please scan and e-mail this report to us as soon as possible so we can add you to our insurance prior to your arrival.



Driver's License scan (all Fulltime team members)

All fulltime team members must submit a scan of your driver’s license, both front & back.  This information is required for for us to obtain security clearance for you at prisons and detention centers where you will be minsitering, and we often need to secure this clearance prior to your arrival.

Please scan both sides of your license e-mail to us as soon as possible.



Health Insurance card (all Fulltime team members)

As stated in the Guide to the CTI Fulltime Commitment, all fulltime team members are required to carry health insurance. Please scan and e-mail a copy of your insurance card, front and back.



Minor Consent Letter

If you are under 18, we will send you a letter that must be signed by both your parents and notarized.  This letter is required by government and airline officials to permit a minor to travel in the custody of someone other than his or her parents.  The physically notarized copy must be returned to us - it cannot be scanned and e-mailed.

Yellow Fever Vaccination Certificate

In certain cases, governments will require arriving passengers to show proof that they have been vaccinated against Yellow Fever.  If this requirement will apply to your tour, we will inform you directly.  A certificate will be issued to you when you are vaccinated, and you will need to provide us with the original certificate.



Visa Application Form

In certain cases,  a visa may be required in order to for you to enter the country where your team will be ministering.  We will handle this for you, but we may need you to fill out some information in advance.  In such a case, we will either send you a physical form to fill out and sign, or we will direct you to a webform.  

If we have not informed you of this requirement, it does not apply to you.
